If you have experienced or witnessed a violation of IU's Discrimination, Harassment, or Sexual Misconduct Policy by an employee, student, or visitor at IUPUI, IU Fort Wayne, IUPUC, or IU School of Medicine, please contact the
IUPUI Office of Institutional Equity.

Make A Report

Report Faculty / Staff / Student / Visitor Conduct to OIE

Make a report to the Office of Institutional Equity regarding discrimination, harassment, sexual misconduct, or retaliation by faculty, staff, students, or visitors.

Report Student Conduct

Report personal misconduct by a student to the Office of Student Conduct in the Division of Student Affairs.

Report Activities to EthicsPoint

Report activities related to financial, research, fraud, information technology, or other policy violations through IU's EthicsPoint.

Report Bias

If you have experienced bias or hate, please report to the Indiana University Incident reporting form.

File a Human Resources Grievance

If you want to report an allegation regarding a Human Resources matter, please fill out a grievance here.

Reporting Concerning Behavior

If you want to report concerning behavior of a student, faculty, or staff member, fill out a Behavioral Consultation Team (BCT) form to connect with the team.
